What
to Do In Time of Bereavement
If Death Occurs At Home:
-
Telephone the Doctor. He will visit to
confirm death has taken place

Contact a funeral director
-
Collect death certificate from the Practice
if this can be issued otherwise you will be given other instructions

Please inform the Practice if cremation is
planned
When Death Occurs In Hospital:
-
Contact a funeral director to inform them
that their services will be required
-
Collect Doctor's death certificate from the
hospital
-
Take this to the registrar's for the area in
which death took place. Also take the deceased's medical card if available
and birth certificate
-
Take green form to funeral director who will
take over complete responsibility for arranging the funeral
